Special Issue ACCC's "Transitions between Care Settings 'Best Practices' Project" to include:

  • Highlights of barriers, solutions, and best practices identified in the "Transitions between Care Settings" project.
  • Mini-profiles of the 9 ACCC-member programs (5 hospitals and 4 practices( identified by the project survey analysis as having exemplary transition practices in place and the tools these programs have agreed to share.
