Essential Responsibilities:
We are looking for an experienced Social Worker to assure coordination and continuity of care for adult oncology patients and their families through our Cancer Center. This is a full-time, day shift position with twice-monthly evening bereavement support meeting facilitation. Services include, but are not limited to:

  • assessing patient/family strengths in regards to social, environmental, psychological, financial, ethnic, spiritual, and cultural influences related to living with cancer;
  • enhancing client access to resources;
  • providing one-to-one emotional support to patients and families;
  • facilitating groups and family conferences; participating in patient care planning rounds;
  • and supporting continuity of care with the Overlake Hospital medical staff and other healthcare professionals.

Essential Requirements:
Requires Bachelor's degree in Social Work and registration as a WA State counselor. Master's degree preferred. Two or more years experience serving patients and families dealing with a diagnosis of cancer, preferably in an outpatient healthcare setting, is desired.
